Community Health Workers provide support to chronically ill participants to improve their health outcomes and work to help lower health care costs for providers in the area.
- Community Health Workers (CHWs) identify the needs of participants and make referrals to resources within the community.
- CHWs connect participants to insurance and a Primary Care Physician
- Check on medication and appointment adherence, educate participants on proper ED usage and their chronic illness, review and update self-management goals and discuss issues that may be affecting their lives at no cost to the participant.
