1. We connect participants to resources such as:

- Primary Care
- Transportation
- Legal Aid
- Food
- Housing
- Medication
- Employment
2. We work with participants to serve as a link between primary care and the community.
3. We help connect and educate participants on a variety of topics including pregnancy, chronic illness, and medication adherence
4. We help participants communicate and interact with their primary care provider.
