We’d love the opportunity to share more with you about our Community Health Worker (CHW) program created to improve health outcomes by connecting residents with the health services and resources they need most.
Many of you have expressed interest in bringing a CHW into your agency to better support the clients you serve. Rather than managing the costs and logistics of hiring a CHW on your own, we invite you to consider partnering with Access Health.
