Community Health Workers work as part of a health care team. 

Community Health Workers understand the health goals of their participants. They communicate regularly with primary care providers the concerns and progress made with each participant. A Community Health Worker serves as a liaison between the community and the health care, government, and social service systems.

These are the people on the front lines, providing advocacy, education, and support to participants to help participants improve their lifestyle and connect them with their proper healthcare options. They collect data and discuss health concerns with the healthcare team.

The strategy is to identify and address risk factors at the individual and community level using collected data.
