Primary Care Pilot Project                                 

Funded by North Canton Medical Foundation, our primary care pilot project focuses on patients with chronic disease, especially those with diabetes.   Our proprietary tools for physician communication and support, reporting, and use of Pathways for health outcomes, are allowing us to see the integration of CHWs into health teams at both Mercy Ambulatory Care Clinic and Aultman’s My Community Health Center.  Connecting patients to diabetes education and fitness and nutrition programs in their neighborhood help with the self-empowerment that our residents need to manage their chronic illness.  Medication education and adherence is extremely important and our Community Health Workers help patients understand their medications and provide reminders about appointments with doctors and specialists.  This project addresses the social determinants of health that lead to poor health outcomes.
