Community Health Workers
As Access Health collaborates with a number of partners and communities around CHW initiatives, we are seeing the larger scale impact on the health and well-being of the county. We aim to lower health costs by reducing ER visits and missed appointments; reducing hospital readmissions; and by increasing the ability of our residents to understand their health and health insurance, and as a result, the ability to adhere to medication regimens set by their doctors.
A relationship with a provider works with one’s health insurance to provide safety for health. We want our residents to be able to be their own health advocate and know the questions to ask in the doctor’s office.
Our Community Health Workers address Infant Mortality as a partner with the THRIVE Initiative. We also work with participants that have one or more chronic diseases. Our partners include Mercy Medical Center Ambulatory Care Center, My Community Health Center and several physician offices throughout Stark County.
